Parent/Guardian Permission to Participate I give permission for my child to participate in Life Ready Lab: A 5-Day Teen Life Skills Bootcamp from June 15–19 at the Hesler Building in Hagåtña, Guam.

Program Activities Acknowledgment

I understand that my child will participate in discussions, group activities, role-play exercises, communication practice, phone call simulations, application practice, interview practice, money management activities, and confidence-building exercises.

Behavior Expectations Agreement

I understand that students are expected to participate respectfully, follow instructions, treat others kindly, and maintain appropriate behavior during the program.

Parent/Guardian Liability Acknowledgment

I understand that reasonable care will be taken to provide a safe and supportive environment. I acknowledge that participation in any program includes some level of risk, and I release itsBarbieJane, LLC, its representatives, facilitators, and partners from liability except in cases of gross negligence or willful misconduct.

Refund Policy Acknowledgment
